Minor Traveling Permit?

Has anyone asked for a letter to authorize the child to go out of the country? It is a letter of authorization from one of the parents (if traveling with the other parent) or both allowing the minor to travel out of the country.

I know you are not supposed to actually do the letter, but does anyone have an idea how it looks/says? I actually need one for myself because I am going out of town with my kids and I was thinking of drawing up the letter myself and just taking to another notary to notarize it (with my husband of course so he could sign it)

I have had also a few people ask me if I do this, but unfortunately have to say no because I have not done one of these. When I come across them again the majority have told me the notary that notarized the letter also wrote the letter him/herself.

HELP! I just don’t want to have to pay double for someone to do the letter, and then another fee to notarize it. And of course, I can’t notarize it myself :)

When I’ve been asked to do this, I suggest that people go on line and Google “Consent to travel with minor.”

There should be a significant number of suitable letters and/or forms you can find on the internet to use/modify to fit your needs.

When doing a letter, it does not need to be formal. You would want to address it To Whom it May Concern, state your name, that you are giving permission for your child (name and age of child) to travel with (name of individual and relationship to child) to travel to (destination) on the dates of (duration of trip), and that the individual has your consent to seek medical attention for your child.

If you really want to get into the whole deal, you can insert the child’s date of birth, birth certificate number or passport number if the child is traveling with a birth certificate or passport. You can insert the name of the airline, dates of flights, time of flights and/or flight numbers.

In my opinion, that’s overkill, but it’s an individual’s preference.

What I’ve found is that people get really nervous and think that this should be a formal, legal and complicated document. That’s not the case.

individual has your consent to seek medical attention for your child.(YOU CAN PUT YOUR CHILD’S MEDICAL RECORD NUMBER IN THE FORM AND/OR NAME OF HEALTHCARE PROVIDER AND INSURANCE COMPANY)

I’ve notarized the signature in a letter where the father gave his wife permission to travel abroad with their daughter. At first, I pulled out the form “Permission for Minor to Travel with Adult” and noticed it had specific dates for travel. He didn’t want that because he wanted his wife to be able to travel anytime and at will. So we forgot about the form and used the letter he typed himself. Of course it had no notarial verbiage because he typed the letter so I asked him which notary act he’d like….Acknowledgment or Jurat. I explained the difference….he picked the Jurat and I completed it and gave him the oath.

LKTHORNTON: I know it may be much but by chance do you actually have a copy of the form “Permission for Minor to Travel with Adult” you mention that you could send me? I have tried googling it and haven’t had any luck finding one. thanks in advance!
